Coaches and Individual Volunteers
Coaches and Volunteers must complete the waivers below. Do you have some questions about blind hockey before you sign up?

Complete the waiver (below) - Each volunteer must sign and submit the waiver on the day of the event chosen to volunteer. If you've chosen to help more than once, only 1 waiver is needed. Every volunteer must be SafeSport certified and has their USA Hockey Background Screen completed.
If you are already USA Hockey Registered, you do not need to fill out the waiver.
